New Patient Registration

1)  Before  you schedule an appointment, please fill out the New Patient Insurance and Demographics Form.  It is very important to fill out the specific information concerning your insurance company name and insurance ID so please have your insurance available when you fill out this form.

 

2)  After about 24 to 48 hours, you will receive an email inviting you to visit your newly created Patient Portal.  

 

3)  Then,  please visit your Patient Portal and fill out your contact information and your Health History form.  This allows Dr. Patterson to be prepared by reviewing your medical history before your visit.  

 

4)  If you have filled out the information above, you may then request an appointment through your portal or by clicking here.

 

5)  Please bring your signed HIPAA Acknowledgement Form to your first visit.

To schedule a new patient appointment with Dr. Patterson, please see the section to the left about the New Patient Insurance and Demographic Form.

 

As noted to the left, it is very important to fill out the specific information concerning your insurance company name and insurance ID, so please have your insurance available when you fill out this form. 

 

Within 24 to 48 hours, you should receive an email inviting you to visit your newly created Patient Portal.

 

If you have already filled out the New Patient Insurance and Demographic Form, visited your newly created Patient Portal, filled out the contact information and Health History forms, and wish to request an appointment, please click here.  

 

Then, please follow the instructions to the left about the HIPAA Information and HIPAA Acknowledgement.

 

 

Established Patients

Dr. Patterson is now using a different portal system and appointment scheduling software.  

 

Check your emails to see if you have been invited to your newly created patient portal. If you do not already have access to your new portal, you can request that she enable your new portal by contacting  her through your existing "Updox" portal which will still be active but will not be the preferred way to communicate with her.   

 

Then to request an appointment, please click here to visit your new portal. 

 

Before that appointment, please be sure to update your Health History forms on your portal.

If you have read and acknowledge your HIPAA rights, please print out the HIPAA Acknowledgement Form  found here, sign it, and bring it with you to your first appointment.

Messaging/Portal

Please be aware that as a solo practioner, Dr. Patterson must streamline her messaging.  In order to do this, she has messaging set up  for you through a "portal"  in her new Electronic Medical Records System that allows communication which is secure and HIPAA compliant.

 

If you would like to send Dr. Patterson messages, sign into your newly created Patient Portal.  You will then have access to private messaging with Dr. Patterson.

 

For urgent matters, please contact Dr. Patterson at 518-779-5225.

 

All other messages should be handled by the messaging system through your portal.  Once you have logged in and sent your message, you can expect a reply within 24 - 36 hours.

Electronic Medical Records

Dr. Patterson will be enabling you to view some of your records online through your newly created Patient Portal.  You will be able to receive visit summaries and view your labs,
