New Patients

1)  Welcome to your new patient portal!  

 

2)  You will now have a patient portal that includes a secure way to send messages, look up lab results and patient care summaries, request appointments, make payments, and update Health Histories. 

 

3)  To request your appointment,  please create your new portal. 

 

4)  You will then be able to request an appointment.

 

5)  As soon as your appointment is scheduled, please complete the Health History form located on your portal's home page underneath the appointment reminder.

Established Patients
